Chronic Conditions: Readmission Rates & Costs
It’s a fact, 1 out of 5 medicare paying patients discharged from a hospital will end back in the hospital within 30 days of discharge. While the causes of each individual readmission can vary greatly, it is accepted that a large percentage of them are preventable. It is no longer acceptable to provide just enough care for a medicare patient to be discharged and on their way to being someone else’s problem in 10, 30, 60, 90 days later. The costs associated with each readmission episode are substantial and CMS has put in place a penalty system to take meaningful action to incentivize providers into improving care quality to reduce preventable re-hospitalizations.
