OverSightMD is a certified Enhanced Care Management (ECM) provider delivering comprehensive, community-based care management to Medi-Cal members across Central and Northern California. What is Enhanced Care Management?
Enhanced Care Management (ECM) is a statewide Medi-Cal benefit established by the California Department of Health Care Services (DHCS). ECM provides equitable, person-centered, community-based care management for Medi-Cal members with the most complex health and social needs.
As a certified ECM provider, OverSightMD assigns each enrolled member a dedicated Lead Care Manager who coordinates all health and health-related care — from primary care and behavioral health to housing assistance and social services — ensuring nothing falls through the cracks.
ECM is provided at no cost to eligible Medi-Cal managed care members.

The Seven Core ECM Services

How ECM Works with OverSightMD
From referral to graduation, our structured process ensures every member receives consistent, high-quality care management.
Referral & Eligibility
Members are referred by their managed care plan, provider, community organization, or through self-referral. OverSightMD verifies eligibility.
Outreach & Enrollment
Our Lead Care Managers conduct culturally competent, in-person outreach to engage the member, build trust, and complete enrollment.
Assessment & Care Plan
A comprehensive biopsychosocial assessment is completed and a person-centered care management plan is developed collaboratively.
Active Care Management
Ongoing coordination of all health and social services, regular monitoring, health promotion, and continuous reassessment of the care plan.
