We Prepare

STEP 01 — BEFORE YOU LEAVE

We partner directly with CDCR facilities across California. We work with your caseworkers and LVNs to transfer your medical records — so your care is ready the moment you return home.



Social Services

STEP 02 — SECURING YOUR BASICS

No waiting in endless lines alone. We directly coordinate your essential benefits: EBT (Food), GA (Cash Assistance), and Medi-Cal (Healthcare).



Therapist Network

STEP 03 — SUPPORT FOR YOUR MIND

The transition can be stressful. We provide dedicated, 1-on-1 therapy through our network — at no cost — so you have someone to talk to as you adjust.

Enhanced Care Management is a no-cost benefit available to eligible Medi-Cal members across Santa Clara, Santa Cruz, Monterey, San Benito, and Merced Counties. Our Justice-Involved Care team specializes in supporting adults transitioning home from incarceration — meeting you where you are with practical, judgment-free help.

You may qualify if you…

ELIGIBILITY SNAPSHOT

✓Are returning to one of our service counties from a CDCR facility, county jail, or juvenile detention
✓Are enrolled in (or eligible for) Medi-Cal coverage in California
✓Have a chronic health condition, behavioral health need, or substance use history
✓Are at risk of homelessness or housing instability after release
✓Need help connecting to primary care, specialists, or prescriptions
✓Want a dedicated advocate for benefits, employment, or court documentation

FREQUENTLY ASKED

? Does this cost me anything?

No. Enhanced Care Management is a covered Medi-Cal benefit. There are no fees, no co-pays, and no paperwork costs for you. Ever.

? Will my information stay confidential?

Yes. Your records and conversations are protected under HIPAA. We do not share your information with parole, probation, or law enforcement without your written consent.

? How quickly can I get connected to a care manager?

We aim to make first contact before your release date when possible, and within 72 hours of any referral after release. Your care manager becomes your single point of contact from day one.
